Obtains and handovers a report of patient progress, ensuring all doctor's orders are followed before changing or over taking shifts and notifying any incidents or problems encountered. 4. Visit patients in the unit with team members to assess conditions, re-plan nursing care, and determine nursing priorities based on patient needs and staff availability. 5. Provides comprehensive hemodialysis patient care using the nursing process approach and various medical devices (hemodialysis machines, CRRT, various access sites), manages all phases of hemodialysis treatment, maintains patient records, and promptly reports abnormalities to medical professionals. 6. Conducts nursing rounds on patients, providing emotional support and accompanying doctors while doing rounds, as well as assisting in procedures as needed. 7. Executing the doctor's orders, coordinating with team members related to patients’ treatment, and fostering effective communication with all disciplines, patients, and relatives to ensure patient satisfaction. 8. Engages actively in health education for patients and their families, unit meetings, and hospital activities, providing support and education to dialysis patients, advising on organ donation programs, and planning treatment programs to ensure adequate treatment. 9. Provide clear, compassionate, and culturally sensitive communication with patients, ensuring their understanding of their condition, treatment options, and care plan in a comprehensive manner. 10. Assist with patient billing procedures, scheduling next appointments and guide them to the cashier for payment processing after receiving treatment. 11. Collaborate with the unit manager in administration, setting care standards, and fulfilling duties in the absence of the unit manager, while also carrying out assigned tasks and providing feedback. 12. Charge accurately, incorporating charges for equipment and consumable items into the patient's charges and verifying their completeness. 13. Inspect and document the supply of sterile packs to ensure the integrity of the packaging and maintain an adequate supply of unit stock from CSSS.

Provide counselling, education, and home program guidance to patients, caregivers, and families to support self-management and continuity of care.5. Work collaboratively with the multidisciplinary rehabilitation team including physiotherapists, speech therapists, doctors, nurses, and rehab aides to ensure coordinated service delivery.6. Support administrative functions such as service operations, scheduling, coordination of rehabilitation activities, and maintaining statistical data for KPIs and performance indicators.7. Participate in staff orientation, supervision, coaching, and mentorship of junior therapists, interns, and rehabilitation assistants; monitor their performance and care delivery.8. Engage actively in continuous professional development (CPD) via training, workshops, competency assessments, privileging processes, and evidence-based practice updates.9. Ensure compliance with KPJ policies, MSQH/JCI/ISO standards, MOH regulations, PDPA requirements, and ethical principles in all clinical and administrative tasks.10. Implement, adhere to, and monitor services SOPs for assessments, interventions, documentation, and quality assurance processes.11. Ensure a safe, hygienic, and functional therapy environment in accordance with hospital safety, infection control, occupational safety and health standards, and emergency procedures.12. Oversee, maintain, and ensure proper functioning of all occupational therapy equipment, including conducting inspections, reporting faults, managing inventory, and ensuring asset safekeeping.13. Participate in quality and safety initiatives including incident reporting, Q-Radar compliance, RCA investigations, continuous quality improvement initiative projects, and promoting a patient-safety culture.14. Serve as a patient advocate, ensuring that therapy services comply with legal, ethical, and institutional standards while maintaining professionalism and positive work culture.15.
