We are looking for a passionate pharmacist to be part of the Yishun Community Hospital (YCH) Pharmacy team. Our YCH team strives to educate, engage and empower our patients through the continuum of care, to eventually integrate them back into community.
Join us to provide patient-centred care by interviewing patients and their caregivers during medication reconciliation. You will care for patients by reviewing their medication regimens for appropriateness and set pharmacotherapy goals. For selected groups of rehabilitative patients, you will assess their abilities to self-administer their medications during their stay in YCH. To empower patients to manage medications independently at home, you will educate them by counselling on the names, indications and common side effects of their medications.
You will be given opportunities to conduct educational sessions with our patients and other healthcare professionals on medication related topics. You will be able to carry out research studies, run ambulatory clinics and review medications at nursing homes. There will also be opportunities for you to mentor and train pre-registration pharmacists and students to foster professional growth for future pharmacists.
