We are looking for a passionate pharmacist to be part of the Yishun Community Hospital (YCH) Pharmacy team. Our YCH team strives to educate, engage and empower our patients through the continuum of care, to eventually integrate them back into community.
Join us to provide patient-centered care by interviewing patients and their caregivers during medication reconciliation. You will care for patients by reviewing their medication regimens for appropriateness and set pharmacotherapy goals. For selected groups of rehabilitative patients, you will assess their abilities to self-administer their medications during their stay in YCH. To empower patients to manage medications independently at home, you will educate them by counselling on the names, indications and common side effects of their medications.
You will be given opportunities to conduct educational sessions with our patients and other healthcare professionals on medication related topics. You will be able to carry out research studies, run ambulatory clinics and review medications at nursing homes. There will also be opportunities for you to mentor and train pre-registration pharmacists and students to foster professional growth for future pharmacists.

Experience as a Pharmacist in relevant clinical settings and demonstrated competency will be an advantage.
Candidates with 4 years of experience can qualify for the Senior Pharmacist position
You will be responsible for the provision of pharmaceutical care by working collaboratively with a multidisciplinary health care team to promote wellness, maintain current health, and/or intervene in acute or chronic illness. You will also identify, resolve and prevent drug-related problems.
