Are you an experienced Medical Social Worker who enjoys working closely with patients, families and healthcare professionals?

We're looking for a dedicated Medical Social Worker to join an established healthcare environment, supporting older adults and individuals with complex care needs.

This opportunity is ideal for someone who brings strong case management expertise, sound clinical judgement and a genuine interest in helping individuals and families navigate important care decisions.

What You'll Enjoy

• Attractive monthly salary of $5,500–$6,500 (commensurate with experience)
• 5-day work week on a rotating roster (Monday to Sunday)
• Working hours: 8.30am – 6.00pm

Your Responsibilities

• Evaluate patients' and families' psychosocial needs and recommend suitable care arrangements.
• Provide professional counselling, emotional guidance and intervention during challenging circumstances.
• Manage complex cases involving elderly individuals, dementia care and family-related concerns.
• Facilitate Advance Care Planning (ACP) and end-of-life care conversations.
• Connect families with appropriate financial assistance, healthcare services and community support programmes.
• Coordinate with medical and allied health teams to ensure continuity of care.
• Support admission assessments, care transitions and discharge arrangements.
• Maintain accurate case records and contribute to service improvement initiatives.

What We're Looking For

• Degree in Social Work or Graduate Diploma in Social Work.
• Registered Social Worker accredited by SWAAB.
• Minimum 3–5 years of relevant experience, preferably in hospitals or community healthcare settings.
• Experience supporting older adults and individuals living with dementia.
• Strong counselling, case management and stakeholder engagement skills.
• Comfortable managing sensitive family discussions and complex care situations.
• Advance Care Planning Facilitator certification is advantageous.
