Job Description

Responsibilities

MOHT envisions a transformed health system that is patient-centric, data-driven and digitally enabled to better empower health, prevent disease and provide excellent value-based care. To realise this vision, MOHT’s mission is to design and implement innovative solutions essential for the desired health system transformation.

The Integrated General Hospital (IGH) programme aims to describe and address key challenges in care fragmentation, particularly for patients with multiple health and/or social issues. Over the past years, IGH has harvested insights on challenges associated with the lack of singular patient ownership, contributed by many providers especially in the community. There are challenges associated with an imbalance of resources, resulting in part from entrenched views that are more hospital-centric and/or organ-centric. There are also challenges around suboptimal transitions of care and poor integration of health and social care. The Care Solutions team within IGH supports a multi-site, evaluation-focused, consensus-driven approach for hospital and transitional care redesign.

The successful candidate will be expected to support priorities and conversations the Care Solutions team engages and manage discrete tasks on a diverse portfolio of data analytics projects. Areas of work include quantitative evaluation of care redesign pilots at various acute and community settings including interventions like home hospitalisation, integrated care, community-based care, chronic disease management, and transitional care. Ideally, the candidate will have experience handling big data using statistical packages and tools. Experience in healthcare sector will be an advantage.
