Responsibilities
Responsibilities:
Identify and engage patients for health screenings
Educate patients on the importance of preventive health and screening.
Coordinate care for patients with the chronic care team.
Perform patient assessments, including vital signs and mental health evaluations.
Document nursing care accurately in compliance with institutional policies.
Make referrals for lifestyle modification programs as needed.
Teach patients how to utilize health monitoring tools at home.
Support health promotion activities in clinics and community settings.
Assist in continuous quality improvement projects related to patient safety.
Requirements:
Diploma holder
Strong commitment to patient care and collaboration with healthcare teams.
Willingness to participate in professional development and training.
Capacity to handle multiple tasks in a dynamic environment.
