Responsibilities
Prepare individualised care and support plans for residents and ensure that identified needs are appropriately addressed.
Carry out the agreed care plans, monitor residents’ progress and maintain accurate records of relevant observations and follow-up.
Develop and coordinate programmes and activities that address residents’ care, social and daily living needs.
Conduct regular resident interviews and prepare six-monthly review reports and monthly records for approximately 50 residents.
