We are looking for a compassionate, proactive, and community-oriented Community Care Project Executive to support the implementation of a caregiver support initiative across our Active Ageing Centres. This role is ideal for individuals passionate about serving seniors, caregivers, and the community through meaningful programmes and volunteer engagement.
As part of the project team, you will work closely with the Project Lead to design, coordinate, and deliver programmes that enhance the well-being of caregivers and care recipients while promoting active community participation.
Key Responsibilities
1. Programme Planning & Delivery
Plan, organise, and facilitate programmes and activities for caregivers and care recipients.
Develop engaging activities that support cognitive stimulation, functional maintenance, social interaction, caregiver education, and respite care.
Ensure programmes align with organisational objectives and community needs.
2. Volunteer Management
Coordinate volunteer-led programmes at Active Ageing Centres and during home visits.
Brief, guide, and support volunteers to ensure safe and effective programme delivery.
Manage volunteer schedules, attendance, and communications.
Assist with volunteer onboarding and training activities.
3. Programme Monitoring & Evaluation
Track participation rates, attendance, and programme outcomes.
Gather feedback from caregivers, seniors, and volunteers.
Prepare reports and recommendations to enhance programme effectiveness.
Support ongoing programme improvements through data collection and analysis.
4. Participant Care & Safety
Monitor participants' well-being during programme activities.
Identify and escalate potential risks or concerns appropriately.
Ensure compliance with safety procedures and established protocols.
Assist in responding to incidents within scope of responsibility.
5. Administrative & Stakeholder Support
Provide operational and administrative support for programme implementation.
Maintain programme records and documentation.
Liaise with caregivers, community partners, volunteers, and stakeholders.
Requirements
Diploma in Community Care, Social Services, Gerontology, Psychology, Nursing, or a related discipline.
Experience in community programmes, eldercare, healthcare, social services, or volunteer coordination will be an advantage.
Strong interpersonal and communication skills with the ability to engage seniors and caregivers sensitively.
Good organisational and administrative skills with attention to detail.
Comfortable working in community settings and interacting with diverse groups.
Proficient in Microsoft Office applications.
