Job Description

Responsibilities

The Case Associate supports the Centre Manager in the coordination, administration and documentation of client services. The role assists in client admissions, care coordination, documentation, reporting and follow-up to ensure timely and quality service delivery. The Case Associate provides administrative support to case management functions and receives professional guidance from the Case Manager on case-related matters where required.

Core Responsibilities

Coordinates client admissions, enrolment, transfers and discharges, including means testing and subsidy applications.

Prepares and maintains accurate client records, reports, and other client-related documentation.

Coordinates Individual Support Plan (ISP) reviews, case conferences, multidisciplinary meetings and caregiver meetings, for tracking and compliance with regulation.

Liaises with caregivers, hospitals, schools, SG Enable, MSF and community partners on client-related matters and follow-up actions, ensuring that they understand the process.

Assists the Case Manager in client assessments, home visits and case reviews where required.

Monitors documentation timelines, follow up on outstanding information and ensures client records are updated.

Prepares reports, claims and documentation required by Management, SG Enable, and relevant agencies within stipulated timelines.

Supports incidents documentation, audits, quality improvement initiatives and service evaluations.

Supports centre programmes, orgranisational projects and any other duties assigned by the Centre Manager.

Core Job Requirements

Education

Diploma in Social Work, Psychology, Social Services or related field

Experience

Fresh graduates are welcomed to apply

Experience in the social service or healthcare sector will be an advantage

IT-proficient and competent with the use of Microsoft Office and client-related system

Skills

Demonstrated skills in organisational and administrative skills

Knowledge of and ability to work effectively with the community and stakeholders

Excellent verbal and written communication skills

Ability to establish and maintain positive working relationships with staff, volunteers, and the public

Must be self-directed and work independently
