Responsibilities
Responsibilities for Social Worker/Caseworker:
Develop individualised care plans based on residents’ needs and conditions.
Implement care plans and monitor residents’ progress and care outcomes.
Develop and coordinate programmes and activities to support residents’ care and well-being needs.
Prepare six-monthly care review reports and conduct monthly interviews for approximately 50 residents.
Qualifications for Social Worker/Caseworker:
Degree or Graduate Diploma in Social Work, Psychology or Counselling.
Willing to adhere to established Standard Operating Procedures (SOPs) when carrying out duties.
Willing to work rotating shifts according to the duty roster.
Minimum 1 year of relevant work experience.
