Key Responsibilities

• Manage medical records filing, tracking, scanning, retrieval, and secure storage.
• Ensure discharged records are complete and uploaded into the EMR system.
• Perform coding using ICD-11 and ICD-9-CM.
• Review records for completeness and follow up with doctors.
• Process medical report requests within required timelines.
• Verify identity and consent before releasing confidential information.
• Maintain confidentiality in line with PDPA 2010.
• Prepare hospital statistics and Ministry of Health reports.
• Support DRG implementation.
• Coordinate EMR downtime drills.
• Serve as committee secretary (agendas, minutes, action items).
• Conduct audits on documentation, coding, and scanning.
• Assist investigations on duplicate records or unauthorized access.
• Support record retention and confidential disposal.
• Train staff on documentation, coding, and confidentiality.
• Assist accreditation and regulatory inspections.
• Perform other duties as assigned.

Qualifications

• Diploma or Bachelor’s in Health Information Management, Medical Records, Health IT, or related field.
• Experience in hospital/healthcare setting preferred.
• Knowledge of medical terminology, coding, and record regulations.
• Familiarity with ICD-11, ICD-9-CM, DRG, and EMR systems is an advantage.
