Job Description

Responsibilities

This role will provide administrative and operational support to ensure the smooth admission and discharge of clients.

Location: Vanguard Care Home (Woodlands)

Client & NOK Engagement Related Scope

  • Attend team meetings to discuss and follow up on administrative care referrals for clients where necessary.
  • Liaise with external organisations that provide services to meet clients’ and their families’ needs, and refer them to community support organisations if necessary.
  • Assist Social Worker to follow up with clients and family members who requires financial assistance.

Administration / Operation

  • Participate as part of the administration team to conduct client’s pre-admission, admission and discharge.
  • Conduct intake assessment including basic financial counselling and service agreement.
  • Establish and confirm source of social facts shown in the social background of all new clients.
  • Support Medifund administration and applications in collaborations with Finance and Social Woker.
  • Support and work closely with Finance to manage arrears.
  • Provide day to day coordination roles and duties between NOKs and Centre.
  • Maintain and upkeep administrative documentation of all clients including filing, tracking and updating where necessary.
  • Maintain and upkeep Centre’s databases
  • Manage and update data into relevant system such as BRIGHT, ILTC and NMTS, InterRai etc. .
  • Any other duties as assigned by the Reporting Manager/Supervisor

Requirements

  • Diploma in relevant field
  • Demonstrate initiatives to identify, plan, develop and implement resourcefulness to find solutions to missing gaps in work processes and mutually benefits stakeholders.
  • Good communications and people engagement skills
  • Good organisation ability
  • Ability to multi-task and eye for details.

Years Of Experience Required

  • Candidates without experience are welcome to apply.
  • At least 1 to 3 years of relevant healthcare industry experience.
