Responsibilities
Coordinate and support the planning, implementation, and evaluation of community care programmes for seniors and caregivers.
Engage clients, caregivers, volunteers, and community partners to facilitate programme participation and service coordination.
Monitor programme outcomes through data collection, reporting, and continuous quality improvement initiatives.
Conduct ground visits and collaborate with internal and external stakeholders to enhance programme delivery and client experience.
Support volunteer recruitment, onboarding, training, and coordination to ensure smooth programme operations.
Manage programme administration, procurement, documentation, audits, grant support, and operational reporting.
Requirement
Diploma/Degree in Healthcare, Social Work, Psychology, Public Health, Business, or related disciplines.
Preferably 2 years of experience in healthcare or community care with strong stakeholder engagement skills.
Proficient in Google Workspace and Microsoft Office applications.
