Location: Central & Western District

Roles & responsibilities:

• Provide service in elderly healthcare specialty and assist the Chief Care Coordinator and the Senior Care Coordinator of the Central & Western District Health Centre (C&W DHC) in offering health assessment, screening, counselling, health education, and other nursing services in the elderly healthcare setting;
• organize both in-centre and community health promotion and primary prevention programs;
• host community education activities, community health assessment services, home visits and others;
• act as case manager of community rehabilitation programs for C&W DHC Members;
• coordinate the care among multi-disciplines in C&W DHC and the C&W DHC Network Service Providers;
• operate the clinical management system for clinical documentation and communication; and
• perform any other duties as assigned.

The ideal candidate should have/be:

• a Certificate of Registration (Part I) issued by the Nursing Council of Hong Kong or equivalent;
• a valid Practising Certificate issued by the Nursing Council of Hong Kong;
• at least 5 years of post-registration experience in patient care related nursing practice as a registered nurse; and
• post-registration training in a clinical specialty is preferred.

Compensation & employment terms:

The successful candidate will be appointed on contract terms with a salary currently at $35,780 per month (Tung Wah Master Pay Scale Point 15) plus a specialty allowance currently at $2,000 and community outreach allowance currently at $2,000. Entry salary depends on years of recognized experience. The salary is subject to downward/upward adjustment or may be frozen as determined by Tung Wah and/or as appropriate with reference to the civil service pay review. The fringe benefits include paid annual leave, medical services, a dental scheme and training sponsorships.
