Job Description

Responsibilities

Job Summary

The role involves proactively engaging clients, organising social activities and encouraging an active lifestyle to prevent social isolation. The position also entails referring clients to relevant community care and social assistance when needed. Under the Age Well SG initiatives, the role requires collaboration with primary care physicians (e.g., family doctors) to empower residents in taking charge of their journey toward healthier and more active lifestyles.

Responsibilities

  • Plan and deliver evidence-based active ageing programmes and activities to promote senior wellness
  • Coordinate befriending and buddying services to enhance client social support
  • Facilitate group activities, community outreach programmes, and engagement initiatives to increase participation
  • Support integration initiatives under AAC-CREST to improve service delivery
  • Recruit, onboard, and coordinate volunteers, matching them to appropriate activities to optimise engagement
  • Build and maintain partnerships with community organisations and stakeholders to expand resource networks
  • Administer new client intakes and conduct non-clinical health assessments to monitor client well-being
  • Monitor and record clients' basic health information accurately for ongoing care management
  • Maintain up-to-date client, volunteer, and programme records using the in-house AAC management system
  • Perform administrative tasks including data collection, report preparation, and programme outcome evaluation
  • Respond to general client enquiries
  • Participate in team meetings, case discussions, and learning opportunities to enhance service quality
  • Undertake additional duties as assigned by the Manager or Executive Director to support organisational goals

Preferred competencies and qualifications

  • Prior experience working in a Senior Activity Centre or Active Ageing Centre
  • Proficiency in MS Office and Google Suite applications
  • Strong communication and interpersonal skills
  • Ability to work independently and collaboratively within a team
  • Passion and commitment to working with seniors
