JOB PURPOSE:
- o provide direct comprehensive nursing care for patients as assigned. This includes supervising the nursing team during his/her shift, providing supportive care, collaborating with other healthcare providers/services, and navigating the total care of patients from admission till discharge.
- To demonstrate professional and evidence – based practices.
- To deputize the Nurse Manager in her absence.
- To work according to established policies, technical and professional standards.
JON CONTENT:
Operational
- Plan, organize, communicate, supervise and evaluate the total care plan for the patients.
- Guide and direct activities of co-workers as they provide patient care.
- Provide supervisory support within the assigned area.
- Triage patient upon arrival and set priorities.
- Observe, record of vital signs of patient and report all abnormality of patient’s condition to physician.
- Assist doctors in examination of patients.
- Ensure doctor’s orders, treatment and investigations are being carried out.
- Administration of medications, record use of drugs and other given treatment.
- Carry out nursing procedures for patients e.g. catheterization and dressing.
- Carry out diagnostic / therapeutic procedure when required e.g. ECG, nebulizer, blood glucose monitoring.
- Performs extended roles when required e.g. IV cannulation, venipuncture.
- Prepare trolley and assist in performing special procedures ************* injection and etc.
- Arrange patient for check-in, transfer to another hospital or for check-out.
- Assist in coordinating of ambulance services to transfer out patients.
- Transfer of ill or potential of deteriorate patients needing hospitalization to hospital by ambulance.
- Check crash cart every shift to ensure all drugs are in place and equipment are in good working order.
Education and Training
- Assist in the orientation of newly appointed nursing staff.
- Supervise student nurses from any College of Nursing and Health Sciences in carrying out basic nursing care for patients.
- Teach patients and relatives according to their needs by formal and informal education.
- Participate in continuing education programme.
Inventory Control
- Maintain control of unit charging and revenue protocols ensuring accuracy and effective compilation of all patient related charges.
- Check consumable and non-consumable par level daily, weekly medicine floor stock check and monthly equipment check depending on the usage frequency.
Maintenance
- Liaise with ancillary services e.g. engineering and housekeeping department for the general upkeep of the unit.
- Liaise with Facilities Department to ensure that all equipments are in good functioning order at all times.
- Ensure assigned weekly and monthly checks on all equipment are being carried out.
Administrative
- Participate and contribute ideas at multi-disciplinary department meetings and other committees towards improvement of the delivery of nursing care.
- Participate in tracking of Quality Indicators to measure care and services rendered to customers and for continuous improvement.
- Compiles and maintains relevant documentation, records and reports for inspection by the appropriate authorities, both internal and external auditors.
- Prepares written or computerized documentation and reports as required by the profession and the department.
- Participates in any assigned task forces, special projects and attends meetings as required.
- Perform other related duties as assigned by the Assistant Manager or Nurse Manager of the unit.
Communication & Relationship
- Provide and receive complex difficult and sensitive information, which may be in a challenging and emotive atmosphere.
- Communicate and collaborate with nurse manager, peers, medical staff, patients, families and carers, general public, external agencies, voluntary agencies, GP's, other regional healthcare.
- Listen to patient and carers to promote and practice shared decision making.
- Collaborate with multi-disciplinary team and representatives to promote service improvement and development
- Prepare and present accurate reports both verbal and written.
- Adopt and oversee accurate, robust and high standard of record keeping.
- Facilitate and discuss multi-disciplinary reviews, daily reviews and handovers.
- Promote and practice a culture of compassion where relationships are based on empathy, kindness, respect and dignity.
- Develop effective interpersonal skills in managing conflict and resolution.
- Adopt a common-sense approach to confidentiality.
- Represent the team at operational, and clinical forums as delegated by the nurse manager to ensure effective two-way communication. Decision & Judgement
- Assess, plan, implement and evaluate care, treatment and risk for service users.
- Recognize potential risk and risk situations and act accordingly in conjunction with IHTRC policies.
- Consolidate clinical theory and practice.
- Implement action plans.
Planning & Organization
- Prioritize work and oversee on a daily basis to meet patient’s needs.
- Contribute to the planning of the future delivery of patient services.
- Adjust staff rosters, ensuring adequate provision of staff across shifts and taking appropriate action when staff numbers are below acceptable levels.
Physical, mental & emotional
Physical:
- Push/ pull trolleys/ wheelchairs.
- Assist patient with personal care (i.e. dressing, hygiene, toilet needs) involving bending, kneeling, and twisting.
- Facilitate and assist patient movement within IHTRC.
- Body mechanism are well maintained throughout patient’s care.
Mental:
- Concentrate on patient’s treatment and care.
- Concentration required when checking documents, clinical notes and administering medication, whilst subject to frequent interruptions from patients / relatives/ team members/ phone calls.
- Unpredictable situation and working schedule requiring immediate change to activity depending on the daily census in IHTRC.
- Coordinate and conduct reviews with a multi-disciplinary team weekly.
- Prepare reports for and attend other meetings as required.
Emotional:
- Deal with distressed/ anxious/ worried/ upset/ angry patients and relatives.
- Occasionally have to care for terminally ill patients.
- Due to nature of different patients group, can be expected to deal with severely challenging behavior.
- Will be expected to impart unwelcome news.
- Provide emotional support to patients and carers.
- Management
- To assist the nursing management team in managerial work such as roster planning and ensuring adequate staffing for smooth operations.
- To assist nurse manager to provide information in activities such as stock inventory, conduct checks and report faulty equipment.
- To provide updates on daily unit operation to management in the absence of the manager.
- To deputized assistant / nurse manager in her absence.
- To assist in infection control activities and to help to ensure infection control prevention measures been complied by the staff all the time.
