Responsibilities

• Conduct casework and provide psychosocial support to patients, caregivers and families

• Review intervention plans and collaborate with all stakeholders including external agencies

• Administer and evaluate efficacy of welfare programmes and develop new initiatives

• Manage patient admission and conduct assessment for financial assistance

• Plan, organize and manage social programmes for patients

• Maintain and update patient records and billing information

• Lead and/or facliitate group work and program that aim to develop target (renal) groups of clients

• Promote programs to target client groups (renal) or other stakeholders/partners/agencies (e.g. understanding of the various services and programs available) and advise stakeholders to undertake relevant programs and services

Requirements

• Degree in social work with SASW accreditation

• Minimum 2 -3 years of relevant experience

• Passion for uplifting patients’ lives through providing holistic social care

• Team player and with good written communication and interpersonal skills
