This role will provide administrative and operational support to ensure the smooth admission and discharge of clients.
Location: Vanguard Care Home (Woodlands)
Client & NOK Engagement Related Scope
Attend team meetings to discuss and follow up on administrative care referrals for clients where necessary.
Liaise with external organisations that provide services to meet clients’ and their families’ needs, and refer them to community support organisations if necessary.
Assist Social Worker to follow up with clients and family members who requires financial assistance.
Administration / Operation
Participate as part of the administration team to conduct client’s pre-admission, admission and discharge.
Conduct intake assessment including basic financial counselling and service agreement.
Establish and confirm source of social facts shown in the social background of all new clients.
Support Medifund administration and applications in collaborations with Finance and Social Woker.
Support and work closely with Finance to manage arrears.
Provide day to day coordination roles and duties between NOKs and Centre.
Maintain and upkeep administrative documentation of all clients including filing, tracking and updating where necessary.
Maintain and upkeep Centre’s databases
Manage and update data into relevant system such as BRIGHT, ILTC and NMTS, InterRai etc. .
Any other duties as assigned by the Reporting Manager/Supervisor
Requirements:
Diploma in relevant field
Demonstrate initiatives to identify, plan, develop and implement resourcefulness to find solutions to missing gaps in work processes and mutually benefits stakeholders.
Good communications and people engagement skills
Good organisation ability
Ability to multi-task and eye for details.
Years of Experience Required:
Candidates without experience are welcome to apply.
At least 1 to 3 years of relevant healthcare industry experience.
