Our multidisciplinary team collaborates to create unified health plans, catering to complex care needs, including stroke recovery, Parkinson's, dementia, and functional decline prevention. Committed to enhancing quality of life, we aim to help individuals thrive rather than merely achieve medical stability. This is a part-time and flexible, on-site role in Singapore for an AHPC-registered Occupational Therapist, Physiotherapist, Speech Therapist, Music Therapist, or Art Therapist with community experience. The therapist will conduct assessments, develop and implement tailored therapy plans, and provide holistic care to clients in diverse environments. The role involves focusing on rehabilitation that addresses individual needs, supporting functional independence, and collaborating closely with a multidisciplinary team to ensure the best outcomes for clients. Qualifications: - Proficiency in Occupational Therapy / Physiotherapy / Speech Therapy / Music Therapy / Art Therapy, Medicine, and Rehabilitation to deliver effective, all-encompassing therapy services. - Strong communication skills to engage with clients, families, and team members effectively. - Experience or expertise in Pediatrics for providing therapy services tailored to children, if applicable. - AHPC registration and relevant professional qualifications in Occupational Therapy, Physiotherapy, Speech Therapy, Music Therapy, or Art Therapy. - Experience in community-based rehabilitation or therapy settings is highly desirable. - Commitment to a client-centered approach and passion for making a meaningful impact on individuals' lives. - Ability to work effectively within a multidisciplinary team environment. Location: Clients' homes or/and Lifeweavers' clinic depending on clients' needs. Education: Bachelor's or equivalent (Required) Experience: Rotated clinical: 7 years (Required) License/Certification: Singapore's Allied Health Professionals Council (Required - Practitioners not registered in Singapore need not apply.)
