Help prevent the next asthma crisis—before it happens.
Join Asthma Virtual Care (AVC) team and become the operational link between patients, digital monitoring tools and the multidisciplinary care team.
Asthma care is often reactive: patients may deteriorate between appointments and return only when they require urgent treatment. AVC aims to break this cycle through continuous digital monitoring of patient reported outcomes, risk-stratified outreach and timely clinical escalation.
You shall lead in transforming patient responses received via chatbot and dashboard alerts into coordinated action—monitoring activity, identifying patients requiring attention, following up with non-responders and activating the appropriate team according to workflow and protocols. You will get the opportunity to engage with patients to seek enrolment (e.g. through phone calls). You will ensure eligible patients are enrolled, interactions and outcomes are accurately documented, and no patient is lost between digital monitoring and clinical care. Through the monitoring of dashboard, follow-up, data collation and analysis, you shall identify trends and care gaps that support safer operations and continuous improvement.
This role is ideal for an individual who has a patient centric mindset, meticulous, organised and enjoys working in a team, connecting people, processes and data. You will work with a multidisciplinary team, contribute to the detection of deterioration earlier, strengthening patient engagement and support in the reduction of avoidable emergency visits and hospital admissions.
Job Responsibilities
1. Outreach to patients to seek enrolment.
2. In charge of the daily monitoring of the dashboard and reviewing patient responses received from the patient fronting chatbot for timely routing and escalation to the CDVC clinical team as per workflow and protocols.
3. Contact non-responsive patients and guide patients to respond using the chatbot where possible.
4. Verify that eligible patients are enrolled and accurately reflected on the dashboard.
5. Update and maintain the patient database timely.
6. Coordinate appointments, referrals, handovers and communication across patients and clinical teams.
7. Ensure accurate, timely documentation of patient interactions, outcomes and escalation across the respective IT systems and applications.
8. Collate and validate data, including enrolment, response rates, patient, outreach attempts, escalations and patient outcomes.
9. Analyse data and identify trends, care gaps, recurring non-response patterns and opportunities to improve operational performance and patient engagement.
10. Prepare regular reports and updates, ensuring information is accurate, complete and readily accessible.
Support audits, service evaluation and continuous improvement initiatives.
Requirements
Qualification Required and Area of Discipline
• A relevant diploma or equivalent experience in healthcare administration, care coordination, nursing support, population health or a related discipline.
• Experience in patient-facing healthcare, telehealth or chronic-disease management is advantageous.
• Familiarity with electronic medical records and multidisciplinary care environments would be beneficial.

• Patient-centred communication: Warm, clear and confident when engaging patients and caregivers by telephone or digital channels. Able to communicate calmly with anxious or unwell patients while maintaining professionalism.

• Data literacy: Comfortable collating and analysing operational or healthcare data using spreadsheets and dashboards.
• Digital confidence: Comfortable with the use and toggling of various IT systems and applications while maintaining accuracy and efficiency.
• Coordination discipline: Highly organised and able to manage multiple patients, deadlines and follow-up actions in accordance with workflow and protocols.
• Attention to detail: Maintains accurate records and follows cases consistently through to completion.
• Collaboration: Able to work independently and effectively in a team setting to achieve the aim of AVC.
• Confidentiality: Handles sensitive patient information responsibly and complies with institutional policies.
• Improvement mindset: Curious, proactive and willing to use data to improve workflows and patient outcomes.
