Job Responsibilities:

Patient Outreach & Enrolment

Contact patients and encourage them to enrol in the programme.
Follow up with patients who do not respond and guide them on how to use the chatbot to submit their responses.
Check patient enrolment status and ensure eligible patients are correctly recorded in the dashboard.



Patient Monitoring & Coordination

Monitor the daily dashboard and review chatbot responses to ensure they are directed to the clinical team promptly when needed.
Follow established procedures to identify cases that require further attention or escalation.
Coordinate appointments, referrals, handovers and communication between patients and healthcare teams.



Data Management & Reporting

Update and maintain accurate patient records and databases.
Record patient interactions, follow-ups, outcomes and escalations in the relevant systems.
Collect and check data on enrolment, response rates, outreach attempts, escalations and patient outcomes.
Analyse data to identify trends, gaps in care, repeated non-responses and areas for improvement.
Prepare regular reports and updates, ensuring information is accurate and up to date.



Audit & Service Improvement

Support audits, service reviews and initiatives to improve work processes and patient engagement.



Job Requirement(s):

- Minimum Diploma or equivalent experience in healthcare administration or related discipline
