GENERAL DESCRIPTION
The In-Charge is overall responsible for the direct management of the activities of the designated service. Monitoring and improving standards of care through supervision of nursing practice, clinical audit, teaching and research. Involves in total nursing management out of normal working hours in the hospital when required.
RESPONSIBLE FOR
Direct supervision and co-ordination of the activities in the allocated unit / ward, in line with the policies and procedures of Nursing Services and hospital.
Responsible for keeping the Chief Nursing Officer and Deputy Chief Nursing
Officer informed of any incidents or reportable situations that need nursing administrative involvement.
JOB DESCRIPTION
1.0 ORGANIZATION AND MANAGEMENT
1. Ensure a high standard of care is delivered in a safe, effective, efficient and caring manner; and organizes, directs and coordinates with the other services in the Facility.
2. The organization chart provides a clear representation of the structure.
3. Vision and Mission Statements, Goals and Objectives suit the scope of the Nursing Services.
4. Works closely with her nursing team, Consultants / Doctors, collaborates with other services / personnel in overseeing patient care is being delivered for optimal client satisfaction. Follows Consultants’ ward round with the team leader and assists in procedures.
5. Participates in the planning, development and evaluation of nursing facilities and services.
6. Updates nursing statistics, performances, activities, improvements and achievements.
7. Conducts nursing rounds on all patients in her unit to ensure patient-focused holistic cares are given and to ascertain for others additional needs.
8. Holds regular staff meetings to discuss issues or matters pertaining to the operations of the Nursing Services.
9. Participate in Head of Service (HOS) meeting chaired by CEO / GM / Operation Manager working towards achieving the same goal.
10. Participates in hospital clinical committees / Hospital Committee.
11. Checks patient’s charges for completeness and charged accordingly.
12. Participates in the health education of patients and their families in the ward.
2.0 HUMAN RESOURCE AND DEVELOPMENT
1. Participate in the recruitment, interviews and selection of staff for the services.
2. Participate in structured orientation program where new nursing staff are briefed on their services and relevant aspects of the Facility to prepare them for their roles and responsibilities.
3. Executes staff development program to provide the knowledge and skills required for staff to maintain competency and meet the organizational needs and pursue their personal and professional growth.
4. Ensures written and dated job descriptions explained and given to all nursing staff.
5. Conducts written evaluation of staff performance at the completion of the probationary period and annually.
6. Establishes a mechanism for credentialing and the delineation of privileges for specific procedures for nurses in the respective services.
7. Encourages staff to critically examine their nursing practices and continue to pursue educational programs.
8. Coordinates with the In-service Education Unit for clinical training in the services.
9. Maintains a record of staff sick leave, notes significant trends in sick leave and prepares sick leaves statistic monthly or annually as required.
10. Guides the staff in solving complaints concerning patients, relatives and staff using the appropriate communication channels.
11. Counsels staff that habitually violate Hospital Policies and Procedures or not comply with nursing standards and code of ethics before recommending appropriate disciplines.
3.0 POLICIES AND PROCEDURES
1. Reviews policies and procedure periodically and 3 yearly to ensure they reflect current standards of nursing services and practices, relevant regulations, requirements of statutory authorities, and the purposes of the services.
2. Ensures all staff comply to all policies and procedures of the quality and safety systems, e.g., MS ISO 9001:2008, ISO 14001, OHSAS 18001, Patient Safety Goals and National Nursing Audits, and any other quality system that the hospital subscribes to e.g., Malaysian Hospital Accreditation Standards, Joint Commission International Accreditation Standards, HACCP Netherlands, etc.
3. Ensures all staff comply with Prevention and Control of Infection including implementation of surveillance activity in the unit and evidence of involvement.
4. Conducts regular checks on DDA and Psychotropic drugs management.
4.0 FACILITIES AND EQUIPMENT
1. Ensures all staffs maintain records of equipment and inventory for medical supplies and drugs (with Pharmacist’s assistance) and kept in the proper places.
2. Ensures emergency trolley is being checked as stipulated policy and audit at regular intervals for compliance.
3. Ensures all biomedical equipment is calibrated and serviced according to the service contract by working together with the Biomedical person in charge.
4. Involves in the Service Budget including purchase of new items.
5. Provides on the job training / guidance to new staff including cleaning of medical equipment
5.0 SAFETY AND QUALITY IMPROVEMENT ACTIVITIES
1. Fully receptive and supportive in the activities that contribute towards continuous improvement of nursing service e.g., Quality Assurance, Infection Control, Fire and Safety Program.
2. Monitors the quality of care / service in designated units and subsequently initiates action and implementation of the corrective measures.
3. conducts investigation and root cause analysis in any incident in the services, takes appropriate action and ensures completion of incident reports within stipulated time frame.
4. Promotes and comply with the Baby Friendly Hospital Initiative, Breastfeeding Policy, etc.
5. Participates in 5S activities and comply with the specified requirements.
6. Ensure all staff always adheres to Standard People Practice (SPP).
7. Adopt and practice the Patient Safety Goals as safety improvement measures recommended by World Alliance for Patient Safety (2008).
8. Carries out duties as an employee as stipulated in Occupational Safety and Health Act and Regulations 1994: -
a. To take reasonable care at work for the safety of yourself and other people.
b. To co-operate with your employer or any other person in the discharge of any duty.
c. To wear or use any protective equipment or clothing provided by the employer.

6.0. ADDITIONAL DUTIES AND RESPONSIBILITIES IN THE SPECIFIC AREA
Senior Specialist Nurse In-Charge
1. Trained and certified for BLS/ ACLS.
2. Ensures good skill mix in the unit and enough staff to administer care.
3. Privileges all trained staff without basic post, performs core competency assessment yearly and repeat privileging every 3 years.
4. Ensures staff has sound knowledge and experience in the management of patients for:
Gastro intestinal related procedures e.g., Esophagus Duodenum-Gastroscopy, Colonoscopy, Cystoscopy and related therapeutic procedures
Cardiac related procedure e.g., Angiogram, Angioplasty, Electrophysiology Study, Echo-cardiogram, Stress test
Respiratory related procedure e.g., Lung Function Test
Geriatric nursing and wound care
5. Ensures effective and appropriate triaging conducted for all patients.
6. Responsible to ensure that all scope cleaning process in the unit are in accordance to standard practices
7. Coordinates and liaise with other services and external agencies when require to ensure safe and smooth transfer of the patient to other facilities.
8. Coordinates with consultant and staff in prioritizing cases.
