1. Clinical Pharmacy Services

· To review and validate medication orders for accuracy, appropriateness, and safety.

· To dispense medications according to physicians' prescriptions.

· To monitor patients for drug interactions, allergies, contraindications and adverse drug reactions.

· T provide medication counselling to patients and caregivers.

· To participate in multidisciplinary ward rounds where applicable.

2. Medication Management

· To ensure the safe storage, handling and dispensing of medications.

· To monitor medication expiry dates and remove expired or damaged stock.

· To maintain adequate medication inventory and stock levels.

· To coordinate procurement of pharmaceuticals and medical consumables.

· To ensure availability of emergency medications.

3. Pharmacy Operations

· To establish and implement pharmacy workflows and standard operating procedures (SOPs).

· To develop medication formulates in collaboration with hospital management and medical staff.

· To maintain accurate dispensing and inventory records.

· To ensure proper documentation of controlled medicines.

· To supervise pharmacy assistants and support staff.

4. Regulatory Compliance

· To ensure compliance with applicable pharmacy laws, regulations, accreditation standards, and hospital policies.

· To maintain records required by regulatory authorities.

· To participate in internal audits and quality improvement activities.

· To report medication errors, adverse drug reactions, and near misses.

· To assist in preparing the hospital for inspections and accreditation.

5. Medication Safety

· To promote safe medication practices throughout the hospital.

· To develop medication safety policies.

· To conduct medication reconciliation where applicable.

· To educate healthcare professionals on safe medication use.

· To investigate medication incidents and recommend corrective actions.

6. Education & Training

· To provide drug information to doctors, nurses, and allied health professionals.

· To conduct in-service education on medications and pharmacy-related topics.

· To orient new staff on medication management procedures.

· To support continuous professional development activities.

7. Inventory & Financial Management

· To monitor pharmaceutical expenditure.

· To minimize medication wastage.

· To forecast medication requirements.

· To assist in budgeting for pharmacy supplies.

· To evaluate new medications for formula inclusion.

8. Administrative Duties

· To participate in hospital committees as assigned.

· To prepare monthly pharmacy reports.

· To maintain pharmacy statistics.

· To support implementation of electronic medical record (EMR) or pharmacy information systems.

· To perform other duties assigned by management.

Pay: Up to RM5,000.00 per month
