STATEMENT OF PURPOSE
Provide support in case management through collection of biodata of patient/family through interviews and to provide social information to the Medical Social Worker to facilitate intervention in management of patient care. Manage routine cases assigned by Medical Social Workers or Senior Social Work Assistant (team leader) with minimum supervision from MSWs/Senior SWA.
MAJOR DUTIES AND RESPONSIBILITIES
Gather social information and do preliminary investigation on patient’s needs to facilitate intervention by Medical Social Worker.
Conduct financial assessment on applications for Medifund, Medication Assistance Fund, Remission and Means Testing etc.
Make field visits and accompany patient to return to his/her own home or other social organisation.
Locate home address and trace relatives of patient with unknown identity.
Interpret for Medical Social Worker.
Make payments to patients as authorised by the Medical Social Worker.
Document biodata of patient, interviews, telephone calls, payments and field visits for records.
Register cases, type letters and reports.
Assist in filing if assigned by Supervisor/Manager of Department
Participate in projects that improve the work processes in department/hospital.
Any other duties as assigned by the Supervisor / Manager of Department.
JOB REQUIREMENTS
Diploma preferably with at least one year of working experience in social service/healthcare setting.
