Conduct assessments, follow-up interventions and case management for clients with mental health conditions and/or dementia.

Provide home visits, caregiver support and psychoeducation to clients and their caregivers.

Plan and facilitate cognitive stimulation, reminiscence and other structured activities based on clients’ needs and interests.

Support community mental health outreach, preventive programmes and public education initiatives.

Monitor clients’ progress, provide appropriate support and escalate concerns to the Social Worker when required.

Build partnerships with hospitals, Social Service Agencies (SSAs), grassroots organisations and community partners to strengthen mental health support and awareness.

Requirements

Diploma in Social Work, Counselling, Psychology, Healthcare, Community Care or a related discipline.

Preferably 2–3 years of experience in healthcare, community care or social services, particularly supporting mental health clients and caregivers.

Strong interpersonal and communication skills, with good organisational skills and the ability to work independently and as part of a multidisciplinary team.
