Social Work Associate / Senior Administrator (Integrated Community Care Office)

Job Summary

Presbyterian Community Social Services (PCS) seeks a Social Work Associate / Senior Administrator to coordinate integrated care for seniors across Centre-based and Home-based services. This role bridges community care, operations, and partnerships to enhance senior support networks and service delivery.

Responsibilities

Lead administrative coordination for assigned sub-regions to strengthen community support networks for seniors by collaborating with internal and external stakeholders

Organise programmes and events that engage seniors and community partners to foster active ageing

Develop and refine workflows and processes to improve operational efficiency in community care services

Conduct care assessments for referred seniors and develop person-centred care plans tailored to individual needs

Coordinate care across health, social, and community resources to ensure seamless service delivery

Analyse data and feedback from partners and seniors to identify trends, service gaps, and emerging needs for service review and programme refinement

Manage administrative tasks including organising meetings, taking minutes, and preparing progress reports to support team operations

Collaborate with other community care organisations to close service gaps through outreach and resource linkage for vulnerable seniors

Administer the Emergency Relief Scheme (ERS) for referred clients to provide timely support

Strengthen staff and community asset capabilities within PCS’ Active Ageing Centres, Senior Care Centres, and Home Care services through training and development initiatives

Undertake additional duties and cross-functional projects as assigned to support organisational goals

Preferred competencies and qualifications

Diploma in Social Work or Integrated Community Care

At least 4-5 years of experience working with seniors or in the community care sector

Strong administrative and organisational skills demonstrated by effective programme logistics management

Effective interpersonal, communication, and stakeholder engagement skills to build rapport with seniors and community partners

Ability to work independently while contributing effectively within a team environment
