Job Description

Responsibilities

The Hong Kong Society for Rehabilitation (hereunder as ‘HKSR’) is a pioneer in rehabilitation

dedicated to providing holistic and personalized services to empower persons with disabilities,

chronic illnesses or challenges of aging to lead fulfilling lives, and to advocate for an inclusive society.

We are a non-governmental organization receiving subvention from government and funds from

private donors and other sources. We invite people with the heart and passion to join us in realizing

our social mission.

District Health Centre (DHC) aims to provide primary healthcare services including health promotion,

assessment and patient empowerment program to enhance the public awareness on disease prevention

and the capacity in the self-management of health through the public-private partnership.

Care Coordinator (Eastern District Health Centre)

Key responsibilities:

  • Plan and deliver health assessment and complication screening service, patient empowerment

programs and individual health coaching for DHC

  • Network with different medical and community partners to develop and implement the primary

health promotion services

  • Assist the Care Coordinator (team leader) to monitor the systems for documentation,

communication and service implementation with the medical and community partners

  • Develop education resources and conduct training to the health promoters for provision of

quality service

  • Carry out any other duties or ad hoc tasks as assigned by Supervisor

Requirements:

  • Degree holder or above in nursing discipline, Registered Nurse
  • Minimum 5 years work experience in patient nursing care, especially in patient education and

community health nursing

  • Excellent communication and interpersonal skills
  • Proficiency in use of IT Technology
  • Competency in stakeholder engagement
  • Good command of written and spoken English, Cantonese and Mandarin
  • Shift duty including 1-2 days night shift until 9:00 p.m. per week is required

Work Location: Eastern District

The successful candidate will normally be appointed on contract terms. Renewal of contract will be

subject to the service/operational needs of the department and the candidate’s performance.
