Academic:

• Diploma/Degree in Nursing
• Registered nurse (RN) with valid APC from Malaysian Nursing Board
• Post basic in Palliative Care / Paediatric will have an added advantage
• Experienced in providing home hospice services will have an added advantage

Requisites:

• Minimum 5 years of experience in nursing
• Excellent clinical skills, critical thinking and problem-solving abilities
• Strong communication and interpersonal skills to effectively interact with patients,
• families and the healthcare team.
• Self-driven and the ability to work independently and support clinical team
• Commitment to providing high-quality, compassionate patient care
• Must have own transport to travel within KL and Klang Valley

Key Responsibilities:

1. Home Visit and Care Coordination

• Provide home-based nursing care to patients under hospice/palliative care
• Assess patients’ physical emotion and psychosocial needs
• Manage pain and other symptoms during home visit
• Administer medications and provide treatment as prescribed
• Monitor patients’ condition and document care provided
• Educate and support family members/caregivers on patient care
• Assist with end-of-life care and provide comfort measures
• Participate in bereavement follow up where applicable
• Respond to urgent care needs and changes in patient condition
• Maintain patients’ dignity, confidentiality and respect at all time.
• Collaborate with doctors, social worker and other healthcare professionals in the hospice team
• Participate in multidisciplinary team meetings across hospital.

2. Advocacy and Training

• Conduct education training for healthcare and non-healthcare personnel that is related to Paediatric Palliative Care.
• Conduct advocacy activity that is related to Paediatric Palliative Care.
• Provide training programme for hospice staffs, trainee and volunteers.

Pay: RM4,000.00 - RM5,000.00 per month
