Responsibilities

CENTRE MANAGER, INTEGRATED COMMUNITY CARE PROVIDER (ICCP)

We are looking for a Centre Manager (ICCP) to lead our Integrated Community Care Provider (ICCP) services, ensuring effective care coordination, strong partnerships, and high-quality service delivery across four sub-regions.

Key Responsibilities

· Lead the operations and delivery of ICCP services across assigned sub-regions.

· Manage and develop the ICCP team, providing supervision, coaching, and performance management.

· Build strong partnerships with AIC, MOH, healthcare providers, and community partners.

· Oversee referrals, care coordination, service quality, KPI monitoring, and reporting.

· Ensure compliance with funding, governance, audit, and documentation requirements.

· Develop and review SOPs, support programme improvements, and lead service implementation initiatives.

· Support new programme start-ups and RFP submissions.

· Represent REACH at sector meetings, committees, and stakeholder engagements.

Requirements

· Recognised degree that qualifies the candidate to work in Singapore in Healthcare, Social Work, Allied Health, Community Care, or a related discipline.

· At least 3 years of experience in care coordination, case management, or community care programme management.

· At least 3 years of supervisory or people management experience.

· Experience managing AIC, MOH, or NCSS-funded programmes.

· Knowledge of community care operations, stakeholder engagement, and programme governance.

· Strong leadership, communication, analytical, and organisational skills.

· Proficient in Microsoft Office 365 and data reporting tools. Experience with SharePoint, Caritahub, BRIGHT, or similar systems is advantageous.

· InterRAI Assessor certification is preferred.
