Job Description

The Community Facilitator (ELP and Hostel) is to assist the Manager, ELP and Hostel to provide high quality of daily care to the ELP and Hostel residents/clients. He/She is to supervise and ensure the well-being of the residents/clients with intellectual disability and/or autism. He/She is also to provide training to enhance the development of the residents/clients in the area of independent living skills, leisure, recreation, sports and career development and participation in the community

Support ELP and Hostel Residents

  • Work closely with Life Coaches to support residents and clients to live independently in ELP onsite units.
  • Work closely with Community Facilitators and Training Officers to support Hostel residents in their daily living and training.
  • Facilitate community support network and social/recreation activities.
  • Maintain and ensure the proper records, care plans, and incidents pertaining to residents and clients.
  • Ensure that residents' and clients' diet restrictions and/or medical requirements are adhered to.
  • Ensure that residents and clients are safe within their living environment, and the place is kept clean and tidy.
  • Monitor the health and hygiene factors of the residents and clients.
  • Prepare, monitor and review the Individual Care Plan of residents and clients in ELP and Hostel.
  • Conduct regular visits to ELP residents and clients, and check that housemates live in harmony.
  • Respond to residents/clients' conflicts, emergencies and request for assistance.

Training of Residents/Clients

  • Assess needs, plan and conduct person-centred training according to the care/training plans geared towards achieving person centred goals set with clients and caregivers.
  • Plan and implement programs/activities for the residents in consultation with other staff/AHPs.
  • Supervise, train and monitor residents/clients in area of independent living skills, leisure, recreation, sports and career advancement.
  • Maintain residents/clients independent living skills, and to improve whenever possible.
  • Put in place measures to mitigate any risk factors together with residents/clients.
  • Ensure that monitoring and alert systems in place are in working condition.

Family, Community and Case Work

  • Work with Multi-Disciplinary Team to conduct regular case reviews and discussed concerns and progress of residents/clients.
  • Build rapport and professional relationships with family caregivers and any community stakeholders e.g. employers, grassroots, volunteers/befrienders.
  • Work with caregivers and social worker in family work and future care plan.
  • Guide residents/clients in navigating resources, financial management skills and travelling to their work/appointments and activities to ensure safe transfer.
  • Guide and provide residents/clients with information on social support services available at other organisations and in the community.

Qualification & Skills

  • Degree, or Diploma in any discipline.
  • Min 3 years of relevant working experience.
  • Knowledge and experience in residential, assisted living, home based services, or disability service.
  • Skills in planning and implementing community activity and training programmes.
  • Proficient in computer literacy skills - MS Words, Excel and PowerPoint.
  • Good interpersonal, communication skills and a good team player.

Other Information

  • Able to commit to a 5-day work week on rotating shifts basis (morning shift/ afternoon shift), alternate Saturday/Sunday and public holidays.
