Responsibilities

Overall Job Responsibility

The Service Coordination and Management Division provides dedicated help to persons with disabilities and caregivers who require disability support and services.

We enhance clients’ service journey and ensure that children with developmental needs (CDN) and their caregivers receive dedicated help through providing person-centered support and service coordination. We support caregivers in the referral process for their CDNs to gain access to appropriate Early Intervention (EI) services and community partners.

The Assistant Manager/Senior Executive provides operational and administrative support to ensure smooth and efficient operations and service delivery.

Duties and Responsibilities

  • Service Coordination and Advice to Caregivers/Families of Children with Developmental Needs (CDN)
  • Assess and establish needs of CDNs and their caregivers/families, provide information and advice on services, programmes or schemes suited to their needs
  • Process referrals to government-funded Early Intervention (EI) services
  • Provide case coordination support for CDNs from complex family circumstances, including referrals to other social services
  • Case Coordination and Stakeholder Engagement
  • Ensure seamless and efficient transfer of relevant information to agencies involved in the CDN’s case
  • Organise and facilitate multi-disciplinary case discussions
  • Service Delivery Reviews
  • Participate in project teams to enhance work processes and practices
  • Engage relevant stakeholders (e.g. Government agencies, social service agencies) to identify and address service gaps

Qualifications, Knowledge And Experience

  • Bachelor’s degree in any discipline. Relevant qualifications in social work, counselling, psychology or occupational therapy will be an advantage
  • Preferably 1 year of experience in the social service, early intervention or healthcare sector
  • Passion to serve people with disabilities and their caregivers
  • Good understanding of person and family-centered approaches
  • Strong communication and engagement skills
  • Collaborative and a team player
  • Conversant in English and mother tongue language(s)
  • Proficient in MS Office and preferably CRM systems
