12 months contract
Job Scope:
Manage assigned cases by supporting case coordination and tracking
Engage clients and relevant professionals to gather information and facilitate follow-ups
Coordinate with internal officers and external agencies on case triage and referrals
Provide administrative support for case closure, documentation, and transfers to community partners
Draft clear and professional emails, referral forms, and closure reports
Maintain accurate records by updating case details and uploading documents in the Case Management System
Ensure all cases are handled in accordance with established procedures and timelines
Requirements:
Diploma in Social Science or related fields
Meticulous and experience in handling data
Proficient in Microsoft Office (Excel, Word)
Able to communicate well with internal and external stakeholders
