Job Description

  • Conduct financial assessments and assist clients with applications for financial schemes

  • Collaborate with multi-disciplinary team members to ensure access to care services & financial schemes such as SMF

  • Perform Information & Referral, basic case management and basic counselling support functions

  • Conduct home visit independently or joint visit with MSWs or Home Care team where necessary

  • Conduct care assessment (e.g. InterRAI) for referrals from community with HNF's presence, as assigned

  • Develop care plans and coordinate follow-up actions to address seniors' needs

  • Work closely with healthcare or community partners to support smooth care transitions and delivery of community care services

  • Support evaluation efforts for programmes and projects to ensure quality and effectiveness

  • Undertake outreach and publicity efforts to boost enrolment and relevant programmes

  • Support service development, departmental projects, and Centre initiatives as required

  • Support the administrative functions of the department

  • Attend relevant training, supervision sessions, and casework discussions for continued professional growth

  • Perform any other related duties as directed by the reporting officers.

Qualifications

  • Diploma in Social Work or equivalent

  • Proficiency in English and other languages or dialects to communicate to our clients and beneficiaries

  • Experience in means testing and healthcare setting is advantageous but fresh graduate may apply
