Job Description

Responsibilities

Company Overview

St. Hilda's Community Services Centre is a voluntary welfare organisation. We are a Senior Care Centre with a Day Rehabilitative Centre and an independent Drop-In Centre serving the needs of the community in one of Singapore’s early housing estate in the locality of Tanjung Rhu and its surrounds.

Job Summary

You will assist to lead clinical rehabilitation services at the Day Rehab Centre by conducting comprehensive client assessments, developing and implementing individualized care plans, and collaborating with multidisciplinary teams to drive client progress and service innovation. You will also provide assist to supervise junior staff and clinical trainees, contributing to service improvements and partnership projects.

Responsibilities

  • Conduct thorough assessments for all clients referred to the Day Rehab Centre to determine rehabilitation needs and goals
  • Develop and implement individualized care plans and treatment objectives to enhance client functional abilities and support aging in place
  • Collaborate with multidisciplinary team members in regular meetings to review client progress and adjust interventions accordingly
  • Lead the development and execution of new client services and collaborative projects with partner organizations to expand care offerings
  • Supervise, train, and mentor therapy assistants, volunteers, caregivers, and undergraduate therapists to ensure high-quality clinical support and professional growth
  • Drive continuous improvements and service developments by identifying opportunities and implementing effective solutions within the rehabilitation centre
  • Assume leadership responsibilities including clinical supervision and management of junior staff to maintain operational excellence

Required competencies and certifications

  • Degree or Diploma in Occupational Therapy recognised by the Allied Health Professional Council (AHPC)
  • Full registration with the Allied Health Professional Council (AHPC)

Preferred competencies and qualifications

  • Relevant occupational therapy experience including management or leadership roles
  • Strong communication and interpersonal skills to effectively engage clients, families, and multidisciplinary teams
  • Commitment to supporting clients with neurological, musculoskeletal, orthopaedic, or psychogeriatric conditions to improve and maintain their functional independence
