Position: Case Manager

Location: Welfare Home, Pelangi Village

We are seeking a compassionate and dedicated Case Manager to join our team. In this role, you will support destitute men with addiction challenges in their rehabilitation journey and facilitate their successful reintegration into the community.

Key Responsibilities

· Conduct needs assessments and identify appropriate support services for residents.

· Develop and implement holistic care plans tailored to residents’ individual needs.

· Assist residents with applications for relevant services and support schemes in a timely manner.

· Maintain accurate and up-to-date case records, reports, and documentation in accordance with service standards.

· Collaborate closely with operations and nursing teams to ensure coordinated resident care and management.

· Perform administrative duties and other tasks as assigned by management.

· Working hours are primarily office-based, with occasional after-hours duties as required.

Qualifications & Requirements

· Graduate Diploma or Degree in Social Work, Psychology, Counselling, or a related discipline from an accredited institution.

· Strong communication, interpersonal, planning, and coordination skills.

· Good conflict management and negotiation skills.

· Ability to work effectively both independently and as part of a team.

· Strong sense of responsibility, accountability, and cultural sensitivity.

· Experience or training in case assessment, report writing, and analytical skills is preferred.

· Proficiency in multiple languages will be an advantage.
