JOB PURPOSE
- Works as part of the clinical department team under the supervision of Nurse Manager and participates in the delivery of patient centered care, always maintaining a compassionate and professional attitude.
- Act as Hostel in- charge/supervisor including on Weekends and Public Holidays.
- Contributes to the clinical department organization.
- Support registered nurses in the provision of a caring and safe environment.
- The post holder will demonstrate competence in generic and specialist skills as required within a time frame agreed with their manager following the completion of appropriate training and education.
Operational Clinical:
- Gather all data of new patients: Full medical history, past disease, past treatments, current symptoms, current diagnosis, medications, nutrition supplements, diet habits, lifestyle habits, others.
- Take patients through all necessary diagnostic tests.
- Analyze & summarize client case to doctor.
- Get patient treatment plan from doctor and fully understand it.
- Coach patient on the treatment plan.
- Set appointment dates for administering patient's treatment.
- Assist in administering therapies to patients when needed.
- Follow-up with patients on set dates.
- Assist other team-members when necessary.
- Act as first aid in case of medical emergency (Code Blue)
- Coordinate referral of medical emergency and transfer of patient to hospital.
- Other tasks per deemed necessary by management.
Hostel Supervisor:
- Lead the care in Hostel and supervised and guide Healthcare Assistance in the Hostel.
- Enforcement of rules and regulations in the Hostel.
- Provide immediate patient assessment and if required, paramedic activation for medical emergencies in the hostel.
- Facilitate patient transfer for any medical emergencies via ambulance service.
- Responsible for handling Hostel issues and escalate any incidences according to the incident notification matrix.
- Provide medical advice for minor ailments.
- Assist and monitor patient’s compliance in the Hostel.
Inventory Control:
- Maintain control of unit charging and revenue protocols ensuring accuracy and effective compilation of all patient related charges.
- Check consumable and non-consumable par level daily, weekly medicine floor stock check and monthly equipment check depending on the usage frequency.
Maintenance:
- Liaise with ancillary services e.g. engineering and housekeeping department for the general upkeep of the unit.
- Liaise with Facilities Department to ensure that all equipment are in good functioning order at all times.
- Ensure assigned weekly and monthly checks on all equipment are being carried out.
- Administrative
- Participate and contribute ideas within department towards improvement of the delivery of patient’s care.
- Compiles and maintains relevant documentation, records and reports for inspection by the appropriate authorities, both internal and external auditors.
- Prepares written or computerized documentation and reports as required by the profession and the department.
- Participates in any assigned task forces in regards of patient’s care.
- Perform other related duties as assigned by the Nurse Manager of the unit.
Communication & Relationship:
- Provide and receive complex difficult and sensitive information, which may be in a challenging and emotive atmosphere.
- Communicate and collaborate with nurse manager, peers, medical staff, patients, families and carers, public.
- Listen to patients and carers to promote and practice shared decision making.
- Collaborate with nursing team to promote service improvement and development.
- Prepare and present accurate reports both verbal and written.
- Facilitate and discuss departmental reviews, daily reviews and handovers.
- Promote and practice a culture of compassion where relationships are based on empathy, kindness, respect, and dignity.
- Develop effective interpersonal skills in managing conflict and resolution.
- Adopt a common-sense approach to confidentiality.
Decision & Judgement:
- Assess, implement, and evaluate care, treatment, and risk for patients.
- Recognize potential risk and risk situations and act accordingly in conjunction with
- IHT Rehab Centre policies.
- Consolidate clinical theory and practice.
- Implement action plans.
Planning & Organization:
- Prioritize work and oversee daily to meet patient’s needs.
- Contribute to the planning of the future delivery of patient services within nursing department.
- Physical, mental & emotional
Physical:
- Push/ pull trolleys/ wheelchairs.
- Assist patient with personal care (i.e. dressing, hygiene, toilet needs) involving bending, kneeling, and twisting.
- Facilitate and assist patient movement within IHT Rehab Centre.
- Body mechanisms are well maintained throughout patient’s care.
Mental:
- Concentrate on the patient’s treatment and care.
- Concentration required when checking documents, clinical notes and administering medication, whilst subject to frequent interruptions from patients / relatives/ team members/ phone calls
- Unpredictable situation and working schedule requiring immediate change to activity depending on the daily census in IHT Rehab Centre.
- Coordinate and conduct reviews with a multi-disciplinary team weekly.
- Prepare reports for and attend other meetings as required.
Emotional:
- Deal with distressed/ anxious/ worried/ upset/ angry patients and relatives.
- Occasionally must care for terminally ill patients.
- Due to the nature of different patients’ group, can be expected to deal with severely challenging behavior.
- Will be expected to impart unwelcome news.
- Provide emotional support to patients and carers.

Pay: RM1,700.00 - RM3,000.00 per month
