Key Responsibilities

• Conduct needs assessment to identify community needs and coordinate referrals for financial or psychosocial care services.
• Apply for financial assistance, subsidies and grants. 
• Participate in case management.
• Assess client's needs or presenting issues, where necessary provide timely interventions and actions for desired outcomes.
• Collaborate closely with various department staff and agencies to ensure a safe home or social environment for holistic care.
• Home environment safety/hygiene/well-being supportive care and interventions.
• Support community program services for clients and community outreach.
• Provide timely case documentation.
