Case Manager

Responsibilities

• Provide end-to-end case management for clients and their families under the FITRAH Case Management (FCM) programme, including assessment, intervention planning, monitoring and case documentation.
• Collaborate with relevant prison officers and FITRAH volunteers to deliver coordinated and holistic support that addresses clients' reintegration needs.
• Conceptualise, plan and organise programmes and activities, including liaising with internal and external stakeholders on programme implementation matters.
• Support research, data collection and evaluation efforts to assess programme effectiveness and inform service improvements. 
• Participate in organizational initiatives and cross-team support, as required.
