Job Description

Responsibilities

Job Title: Case Manager
Department: Social Work
Location: Apex Harmony Lodge
Reports To: Senior Medical Social Worker & Head of Clinical Division

We are seeking a highly motivated, compassionate and dedicated Case Manager to join our team in providing holistic, person-centred care for residents living with dementia. If you are passionate about making a meaningful difference in the lives of seniors and their families, we would love to hear from you.

Key Responsibilities:

  • Coordinate and oversee individualized care plans for residents with dementia
  • Work closely with a multidisciplinary team (nurses, therapists, doctors, social workers) to ensure quality care. Participate in team meetings
  • Serve as the main point of contact for residents’ families, providing updates and support
  • Monitor residents’ progress and adjust care plans as needed
  • Facilitate admissions, care transitions, and discharge planning
  • Assist the Medical Social Worker with managing workload, conducting audits, preparing reports, and implementing quality improvement initiatives related to social care coordination
  • Ensure compliance with regulatory and documentation standards

Advocate for residents’ psychosocial and emotional well-being Requirements:

  • Degree/Diploma in Nursing, Social Work, Psychology, or a related field
  • Relevant experience in dementia care, case management, or long-term care preferred
  • Strong interpersonal and communication skills
  • Ability to work collaboratively in a multidisciplinary environment
  • Compassionate, patient, and committed to eldercare
  • Proficiency in information technology, including Microsoft Excel, Word, and PowerPoint, is required
