1.Education
• Diploma or Degree in Medical Imaging / Radiography from a recognised institution.
• Registered with the Malaysian Allied Health Professions Council (MAHPC).
2. Knowledge and Experiences:
• Fresh graduates with Diploma or Degree in Medical Imaging / Radiography from a recognized institution are encouraged to apply; however, candidates with prior experience will be highly preferred.
3. Special Skills and Competency.
• Competent in performing general radiography, CT scan, MRI, ultrasound assistance, and interventional imaging procedures (e.g., coronary angiography in Cath Lab).

Responsibilities
GENERAL DESCRIPTION
Accountable for performing professional diagnostic imaging procedures to support accurate diagnosis and treatment, while ensuring patient safety, comfort, and care. Adheres to departmental protocols, radiation safety standards, and regulatory requirements to deliver high-quality imaging services.
RESPONSIBLE FOR
1. Performing professional Diagnostic Imaging procedures safely and accurately to support physicians in patient diagnosis and treatment.
2. Ensuring patient identification, care, comfort, and safety during all imaging procedures, while minimizing radiation exposure in accordance with ALARA principles.
3. Adhering to departmental protocols, hospital policies, and regulatory requirements, including infection prevention and radiation safety standards.
4. Reporting equipment malfunctions, safety concerns, or incidents promptly to the Head of Diagnostic Imaging Services for corrective action.
5. Collaborating with healthcare team members to coordinate workflow and maintain efficient, high-quality imaging services.
JOB DESCRIPTION
1.0 ORGANIZATION AND MANAGEMENT
1. Adheres to the vision, mission, goals, and objectives of the Diagnostic Imaging Services (DIS), performing all duties in accordance with the professional scope of radiography practice.
2. Works assigned shifts as scheduled, ensuring safe, timely, and high-quality imaging services.
3. Immediately reports to the Head of Diagnostic Imaging Services any changes in workflow, equipment issues, or patient safety concerns that may impact service delivery.
4. Verifies patient identification (two identifiers), validates procedure requests, and conducts safety screenings (e.g., MRI safety checklist, Contrast Media Safety Checklist).
5. Collaborates with Radiologists, Medical Officers, Surgeons, Cardiologists, and other clinical staff to deliver accurate diagnostic images and ensure timely reporting of critical findings.
6. Prepares and administers contrast media under Radiologist direction, monitors patients for adverse reactions, and documents accordingly.
7. Provides clear and compassionate communication to patients and families, ensuring privacy, dignity, and cultural sensitivity throughout imaging procedures.
8. Ensures accurate documentation of procedures, patient records, equipment use, and incident reports in compliance with hospital policies and regulatory standards.
9. Provides proper handover of pending cases, patient updates, and equipment issues to the next shift to support workflow continuity.
10. Maintains stock of consumables, sterile packs, and radiation protection devices, coordinating with supply chain/pharmacy for uninterrupted services.
11. Ensures correct billing and charging of procedures, contrast, and consumables, maintaining accountability and transparency.
12. Supports departmental leadership in daily operations, workflow management, equipment maintenance, and deputizing responsibilities during the absence of senior staff.
2.0 HUMAN RESOURCE AND DEVELOPMENT
1. Professional registration & mandatory learning
• Maintain a valid license / registration with the Malaysian Allied Health Professions Council (MAHPC) and proof of status.
• Complete mandatory education, training hours, and CME requirements.
2. Orientation, initial competency and privileging
• Undergo structured orientation and supervised preceptorship upon appointment.
• Demonstrate competency prior to independent practice.
• Maintain documented privileges through credentialing, with revalidation every 3 years or as required.
3. Ongoing competency assessment & re-credentialing
• Undertake annual competency assessments and appraisals.
4. Emergency & procedural readiness
• Participate in drills for MRI emergency, and contrast reaction management.
5. Supervision, teaching & mentorship
• Provide structured orientation, supervision, and mentorship for students, interns, and junior staff.

• Participate in annual appraisals, KPI setting, and career planning with the Head of DIS.
• Support succession planning and specialization opportunities.
7. Quality assurance, audit & research participation
• Contribute to accreditation readiness (IMS, MSQH, JCI) and external audits.

• Wear and maintain dosimetry badges; comply with monitoring limits.
• Attend infection prevention training, safe patient handling programs, and health surveillance.
3.0 POLICIES AND PROCEDURES
1. Policy Review & Compliance
• Participates in the review and revision of Diagnostic Imaging Services policies, procedures, and work instructions.
2. Adherence to Standards
• Complies strictly with hospital policies and procedures in radiation protection, contrast media handling, patient identification, and informed consent.
3. Equipment, PPE, and Radiation Safety Checks
• Performs routine checks of imaging equipment, resuscitation trolley, and radiation safety devices (e.g., lead aprons, thyroid shields, dosimeters) in accordance with departmental schedules.
4. Infection Prevention & Control
• Ensures strict adherence to infection prevention and control measures in imaging areas, including aseptic techniques, appropriate PPE use, equipment disinfection, and participation in surveillance/monitoring activities.
5. Confidentiality & Data Security
• Maintains confidentiality of all patient information, including medical records, imaging results, electronic health records, and PACS data.
• Ensures compliance with hospital policies and privacy regulations.
6. Education & Competency
• Participates in continuous education and training, radiation safety, and emergency preparedness.
• Completes competency assessments to demonstrate ongoing clinical proficiency.
7. Communication & Teamwork
• Practices effective communication with physicians, nurses, and healthcare professionals to support smooth workflow, accurate handovers, and coordinated patient care.
8. Incident Reporting & Safety Monitoring
• Reports and documents incidents, near misses, and adverse events in accordance with hospital quality and patient safety policies.
• Actively participates in root cause analysis and implementation of corrective actions.
9. Medication Safety (Contrast Media)
• Demonstrates accountability in the safe handling of contrast media, including preparation, labelling, storage, and administration under established protocols.
• Monitors patients for adverse reactions and ensures documentation in compliance with hospital medication safety policies.
4.0 FACILITIES AND EQUIPMENT
1. Operates all imaging equipment safely in accordance with manufacturer guidelines, hospital policies, and radiation protection standards.
2. Promptly reports, documents, and escalate any equipment malfunction, fault, or performance deviation to Biomedical / Engineering staff and the Head of Diagnostic Imaging Services.
3. Maintains cleanliness and readiness of equipment by ensuring proper handling, safe storage, and disinfection of accessories after use.
4. Monitors and maintains adequate stock levels of contrast media, consumables, and linen, ensuring items are within expiry dates and stored in line with safety and infection control standards.
5. Participates in scheduled inventory checks with Unit / Finance staff to ensure accurate tracking and documentation of consumables and drugs (where applicable).
6. Ensures radiation protection facilities and devices are available, functional, and used correctly.
7. Provides guidance and supervision to new staff in the safe and effective use of imaging facilities and equipment, reinforcing adherence to safety, infection control, and quality protocols.
5.0 SAFETY AND QUALITY IMPROVEMENT ACTIVITIES
1. Regulatory Compliance
• All radiographic practices must comply with the Atomic Energy Licensing Act (AELA) 1984 (Act 304) and the requirements set by the Atomic Energy Licensing Board (AELB), Malaysia.
2. Radiation Protection and Safety
• Adhere to the ALARA (As Low As Reasonably Achievable) principle to minimize radiation exposure for patients, staff, and the public.
• Utilize appropriate protective equipment whenever clinically indicated.
3. Radiation Safety Program
• Comply with the hospital’s Radiation Protection Program under the supervision of the licensed Radiation Protection Officer (RPO) appointed by AELB.
• Wear personal radiation monitoring devices (OSL badges) at all times during radiographic procedures and ensure timely submission for monitoring.
4. Infection Prevention and Control
• Follow hospital infection prevention and control protocols, including strict hand hygiene, cleaning and disinfection of equipment, and safe handling of patients in isolation.
5. Quality Improvement
• Engage actively in continuous quality improvement activities, including internal audits, peer reviews, incident reporting, and clinical governance meetings.
6. Training and Competency
• Participate in mandatory Continuing Medical Education (CME), radiation safety training, and refresher courses in compliance with MOH and AELB requirements.
• Undergo regular competency assessments to demonstrate adherence to professional standards, safety protocols, and best practices.
6.0 ADDITONAL DUTIES AND RESPONSIBILITIES IN THE SPECIFIC AREA
1. Adherence to Organizational Standards
• Align all responsibilities with the Vision, Mission, Philosophy, Goals, and Objectives of Diagnostic Imaging Services and the hospital.
• Ensure smooth and efficient service delivery through interdepartmental collaboration and effective workflow management.
2. Regulatory and Standards Compliance
• Perform duties in compliance with the Occupational Safety and Health (Amendment) Act 2022, the Atomic Energy Licensing Act 1984 (Act 304), and AELB regulations.
• Ensure radiographic practices meet local, national, and international accreditation standards.
3. Quality Improvement and Accreditation Support
• Actively contribute to departmental initiatives such as 5S, Integrated Management System (IMS), Malaysian Society for Quality in Health (MSQH), and Joint Commission International (JCI) accreditation programs.
4. Operations and Service Delivery
• Assist the Head of Diagnostic Imaging Services in overseeing daily operations, ensuring services are safe, timely, and patient-centered.
• Address complaints, service delays, and non-conformities promptly, recommending improvements for efficiency and quality.
5. Patient Care and Safety
• Perform Diagnostic Imaging procedures with a focus on accuracy, safety, and minimizing repeat exposures.
• Ensure patient comfort, dignity, privacy, and informed consent are maintained at all times.
• Provide clear communication with patients and families, explaining procedures and addressing concerns professionally.
6. Radiation Protection
• Apply the ALARA principle to minimize patient and staff radiation exposure.
• Ensure correct use of protective devices.
• Wear personal radiation monitoring devices (OSL badges) consistently and cooperate with the Radiation Protection Officer (RPO).
7. Equipment Maintenance and Safety
• Operate equipment safely according to manufacturer guidelines and hospital protocols.
• Report equipment malfunctions immediately to Biomedical Engineering and/or RPO.
• Verify completion and documentation of preventive maintenance (PPM) and annual QC testing by licensed providers.
8. Staff Development and Training
• Support the orientation and onboarding of new radiographers, ensuring familiarity with departmental standards and safety requirements.
• Mentor junior staff, interns, and students, fostering professional growth and competency.
• Engage in ongoing education to maintain and upgrade skills, particularly in specialized imaging procedures.
9. Communication and Teamwork
• Collaborate effectively with Radiologists, Medical Officers, Nurses, and other healthcare professionals to ensure coordinated patient care.
• Provide accurate handovers and maintain continuity of care through proper documentation and communication.
10. Incident Reporting and Safety Preparedness
• Report and document all incidents, near-misses, equipment faults, and adverse events in line with hospital policy.
• Participate in root cause analyses, corrective actions, and safety drills (e.g., contrast reaction, fire safety, MRI emergencies, mass casualty).
11. Infection Prevention and Control
• Strictly adhere to infection prevention and control protocols, including PPE use, and equipment disinfection.
• Support surveillance and monitoring programs to ensure a safe environment for patients and staff.
12. Special Task:
• Perform Stock Take Count & Monthly Stock Monitoring
i. Ensure all items are organized and easily accessible for counting.
ii. During physical stock counts, record and track expiry dates throughout the month.
iii. Create a “soon-to-expire” report identifying items with less than 6 months before expiry.
iv. Submit timely reorder requests for replacement items to avoid stock shortages.
• Update Percentage of Dose Length Product (DLP) for CT Brain
i. Record and update the Dose Length Product (DLP) for adult CT Brain scans (patients aged ≥18 years).
ii. Ensure updates are in line with Lampiran 4A and 4B.
iii. Support compliance with radiation dose monitoring and optimization standards.
• Person-In-Charge of General X-Ray Room (Equipment Cleanliness & Readiness)
i. Ensure the X-Ray room, including equipment, patient coach, and changing room, are thoroughly cleaned and properly maintained.
ii. Ensure equipment is visually presentable, in proper working order, and ready for patient use at the start of each shift.
