Job Description

Requirements

  • Coordinate patient care services by liaising with healthcare professionals, patients, and their families to ensure seamless and efficient treatment plans.

  • Schedule and manage patient appointments and follow-up care, ensuring optimal utilization of resources and minimizing wait times.

  • Maintain accurate and up-to-date patient records, including medical history, treatment progress, and communication logs, adhering to confidentiality and data protection policies.

  • Provide clear and compassionate patient education regarding their condition, treatment options, medication, and self-care instructions, empowering them to actively participate in their healthcare journey.

  • Act as a primary point of contact for patients, addressing their concerns, answering questions, and providing emotional support with empathy and understanding.

  • Facilitate communication between patients and the medical team, ensuring that all parties are informed and aligned on care objectives and progress.

  • Assist with patient intake processes, including gathering necessary information, verifying insurance details, and explaining hospital policies and procedures.

  • Monitor patient progress and identify any potential issues or deviations from the care plan, escalating concerns to the appropriate healthcare providers promptly.

  • Collaborate with the administrative team to manage billing, insurance claims, and other financial aspects related to patient care.

  • Contribute to the continuous improvement of patient care services by participating in team meetings, providing feedback, and suggesting process enhancements.

Responsibilities

  • Identify and address potential barriers to care, such as transportation, financial concerns, or language difficulties, and connect patients with appropriate resources.

  • Collaborate with the clinical team to develop and implement individualized care plans tailored to each patient's unique needs and goals.

  • Monitor patient progress and proactively follow up to ensure adherence to treatment plans and address any emerging concerns.

  • Manage patient flow within the clinic, ensuring a positive and welcoming experience from arrival to departure.

  • Contribute to quality improvement initiatives by gathering patient feedback and identifying areas for service enhancement.
