About the Role:
Join our collaborative pharmacy team in delivering patient-centred pharmaceutical care across subacute, rehabilitation, geriatric, dementia, and palliative care settings. This role offers opportunities to apply clinical expertise, work closely with multidisciplinary teams, and optimise medication outcomes for patients transitioning from acute to community care.
Job Description:
- Deliver clinical pharmacy services across inpatient and outpatient settings.
- Optimise medication therapy through medication review, reconciliation, and patient counselling.
- Collaborate with the multidisciplinary team to ensure safe, effective, and evidence-based medication use.
- Support medication safety, quality improvement, and pharmacy operations.
- Mentor pharmacy students and junior staff, where applicable.
- Must be willing to work on weekends and public holidays
Requirements:
- Degree or post-graduate degree in Pharmacy
- Registered Pharmacist with the Singapore Pharmacy Council and a valid practising certificate.
- Strong clinical knowledge, communication, and interpersonal skills.
- Passion for patient-centred care, teamwork, and continuous improvement.
Working Arrangements:
- Participate in a shared roster, including weekends, public holidays, and on-call duties, to support continuity of patient care.
