Job Description

To provide essential case management services to individuals with mental health challenges.

Job Responsibilities

• Perform assessments to determine client needs, health status, and social conditions
• Develop individualized treatment plans, coordinating services, and offering ongoing support to facilitate client recovery
• Collaborate with clients, families, and multidisciplinary teams to develop intervention plans
• Respond to client crises by conducting risk assessments, safety planning, and arranging immediate care to stabilize the situation
• Contributing to quality management and general departmental activities
  
  

Requirements

Educational and Professional Requirements

• Diploma/ Degree holder
• Nursing and allied health background with psychiatric clinical experience.
